  • This is the registration form for the 2025-2026 season of the Hawaii Youth Symphony's Super Strings Beginning & Intermediate Violin Classes at Le Jardin Academy.

    Registration & Financial Aid Deadlines:

    • Financial Aid Deadline is 11:59 pm on Saturday, August 16, 2025
    • Regular Registration Deadline is 4:00 pm on Saturday, August 23, 2025 (Has been extended through 9/5)

    Please read through the following BEFORE you fill out this form.

    You will need the following things to complete this registration form:

    • A recent, clear, front-facing headshot picture of the student registering to upload to this form. The student should NOT be wearing sunglasses, hat or anything covering their face and the picture should be from the shoulders up so we can clearly see their face. This headshot is used for identification purposes and it is important that we have a clear picture of your child for our records.

    • If you are applying for financial aid, you will need to have your most recent copy of your tax return to upload to this form. The return does NOT have to be from this year and can be from a previous year.

    • The date of your child's last DPT OR Tetanus Shot. We need at least the month and year. You can take your best guess on the day.

    This registration form registers your child for the FULL SEASON that starts on September 13, 2025 and runs through May 3, 2026. 

    • Tuition for Super Strings this year is $620.00. This includes all rehearsals, workshops, and concerts/events. 

    • Financial Aid - HYS does offer need-based financial aid and we encourange all students to apply. If you apply for financial aid, you will only need to pay 10% of the tuition now. Please see the financial aid section in this registration for more information.

    • Once the season begins, there will be NO refunds if a student drops out or is expelled from an ensemble due to behavior or attendance issues.

    • If a student pulls out of the program BEFORE the orientation on September 6, 2025, parents can receive a refund of the tuition amount minus a $75 administrative fee.

    Email is our primary means of communication.

    • It is VERY important that the PRIMARY email address you give us on this form is vaild and checked frequently.

    • BEFORE the season starts, all communication will be going to the email listed under "PRIMARY Parent/Guardian" and if we can only send out to ONE email for whatever reason, the email will be sent only to "PRIMARY Parent/Guardian."

    • Once the season starts, HYS weekly emails that are ensemble specific will go out to ALL email addresses on the form (Primary Parent/Guardian, Secondary Parent/Guardian, AND Student Email).

    Schedule of Events & Rehearsals - Look at the schedule below and put all these dates into your calendar. Students should NOT join the ensemble unless they can commit to all concerts and events and are committed to being at rehearsals weekly. Students who are frequently absent slow down the progress for the whole group.

    • You will need to indicate any rehearsals/concerts that you already know you cannot make on this form. 

    • We expect students to give at least 2 weeks notice (unless there is an emergency or illness) if they are going to miss a rehearsal and at least 2 months notice if they are missing a concert/performance.

    • Attendance will be monitored very closely. Please remember you are only allowed up to 2 absences per semester (excused or unexcused). If a student is absent more than this, they can be asked to not perform with the group or be dropped from the program if the conductor determines they are not keeping up with the rest of the class.

    MANDATORY Parent AND Student ONLINE Orientation - Saturday, September 6, 2025, from 1:30-2:15 pm. At least ONE parent must attend this orientation with their child. A link to join the orientation and reminders will be sent out about a week before the orientation date.

    • During the orientation, we will go over important information for the year along with what students need for the class and student expectations.

    Please make sure all the information is accurate for the student enrolling in the program and have your child's headshot picture ready to upload on this device.
  • Please make sure that the student's LEGAL NAME is entered accurately as this is how your child's name will be listed in all programs.

    The BEGINNING Super Strings Violin Class is for students who have less than 1 year or NO experience. The class will start off with the basics of how to hold the violin & bow and how to read music.

    The INTERMEDIATE Super Strings Violin Class is for students with at least 1 year of experience or who have been recommended by the teacher to move from the Beginning Super Strings Violin Class up to this class. Students further their skills on the violin.

  • Instrument Loans

    HYS offers instrument loans to students in our programs at NO COST to families. If you do need to borrow an instrument from us, please select "yes" below. You will be prompted to then fill out an instrument contract.

    If you ALREADY BORROWING a HYS instrument over the summer, and don't need to get a different size, you can fill out the instrument contract and KEEP the instrument you have for the school year.

    Picking-up an Instrument:

    After you have submitted your registration form, you can make an appointment to come to the HYS Office to pick-up an instrument anytime AFTER August 30, 2025. In your confirmation email, you will have instructions on how to make an appointment along with our office location and parking information. 

  • String Instrument Loan Contract

    This instrument contract is for string instrument students who need to borrow an instrument from HYS. Instrument loans are FREE for students in an HYS program, but we do have a limited number of instruments.
  • This instrument contract is good from September 1, 2025 through May 31, 2026. Instruments must be returned to the HYS Office by May 31, 2026, or a late fee will be imposed for instruments returned after that date.

    Please read the following and digitally sign the contract below:

    1. If the instrument, along with all additional equipment, is damaged due to any abuse, with the exception of wear and tear from careful use, I understand I am responsible to pay for the repair or replacement of the instrument.
    2. It is the student’s responsibility to monitor their instrument, bow and equipment’s condition and report any issues to their HYS teacher as soon as possible. Examples of some things to look out for:
      • Strings do break after prolonged use and do need to be replaced. If a student notices a string is fraying or starting to break, they should tell their HYS teacher at the next rehearsal so they can replace it for them.
      • Bows need to not be overtightened when used and be loosened after use and before putting them away in the case to avoid overstretching of the bow hairs. (HYS teacher will go over proper bow handling with students in class.) Occasionally one or two bow hairs will break off, but if a student notices that a lot of their bow hair has come out or they are breaking bow hairs every day, they should notify the HYS teacher at rehearsal.
      • If a student notices a crack or if a seam starts to separate on the instrument or any other type of damage, they should NOT try to fix it on their own. Often this leads to a more costly repair for parents. Please have them show their HYS teacher the issue in class.
    3. If the instrument is lost, I understand I am responsible to pay the full replacement cost as determined by the Hawaii Youth Symphony.
    4. In the event of either damage or loss of the instrument and any additional equipment, I will immediately notify the Hawaii Youth Symphony.
    5. I agree that no person, other than the student named above, will be allowed to handle, or use the instrument.I agree to surrender the above instrument and accessories immediately upon the request of the Hawaii Youth Symphony.

  • Schedule Conflicts

    This section is so you can let us know if you cannot make a rehearsal or concert. If you don't currently have any conflicts, please make sure you put the rehearsal and concert dates/times in your calendar to avoid scheduling things on those days.

    Generally, rehearsals are on SATURDAYS at the following times:

    • 1:00-2:00 pm Beginning Super Strings Violin Class
    • 2:15-3:15 pm Intermediate Super Strings Violin Class

    Right now, these are the major concert/event dates:

    • Sat., 9/6- Online Orientation; 1:30-2:15 pm. All students and at least ONE parent/guardian is expected to attend the online orientation.
    • Sat., 9/13 - First day of classes
    • Sat., 9/27 - Workshop from 11:15-2:30 pm (Location is TBA)
    • Sat., 1/31 OR 2/7 - Workshop from 11:15-2:30 pm (Location is TBA)
    • Sat., 4/25 - Academy Program Spring Concert at Kaimuki HS Auditorium
    • Sun., 5/3 - HYS End-of-Year Family Event (TBA)
  • Financial Aid Application

    Financial Aid Application Deadline is August 16, 2025. NO LATE APPLICATIONS WILL BE ACCEPTED!
  • HYS is proud to be able to offer need-bassed financial aid assistance to students, so that any interested Hawaii student may participate, regardless of financial ability. More information about the Financial Aid program is available at our website.

    Need-based financial aid is made possible through generous donations from individual Hawaii Youth Symphony donors and the Michael Nakasone Legacy Fund In Honor of Youth Symphony II by Olivia de Jane.

    We offer two types of financial aid, Tuition Assistance and Private Lesson Aid, and families can apply for one or both types of aid.

    Tuition Assistance

    Although financial aid awards come in a variety of amounts, depending on demonstrated need, families should expect to pay at least 50% tuition. HYS does not make any full-tuition awards. Financial aid funding is limited, and we are unable to make awards to every applicant.

    Private Lesson Financial Aid

    HYS has funding to help students receive financial aid to take private lessons. This aid comes in the form of a one-time check that families can use to pay for private lessons. Families are responsible for finding a private teacher on their own and making all arrangements with the private teachers for lessons. HYS will ask families to have the private teacher fill out a digital form as proof that the aid was used for lessons for the HYS student.

  • Neighbor Island Students

    • Students who were accepted into one of the Academy Program ensembles and who are flying to Oahu from a neighbor island for rehearsals at least twice a month get an automatic 50% discount on tuition.
      • Use the coupon code: APNI2526 when you get to the payment section of this registration form.
    • You DO NOT need to apply for financial aid UNLESS you want to apply for Private Lesson Financial Aid.
    • If you would like to apply for Private Lesson Financial Aid:
      • Please make sure you click "Yes" for the question, "Would you like to apply for need-based financial aid?"
      • Then click on ONLY "Private Lesson Assistance."
      • You will need to fill out all the financial aid questions and also submit your tax return to be considered for Private Lesson Financial Aid.
  • Financial aid applicants should fill out every item in this section (any incomplete answers or missing documents will not be reviewed) by the Financial Aid Application deadline: August 16, 2025. 

    In the payment section at the end of the form, enter the code FINAIDAP2526. This will allow you to make an initial 10% deposit towards tuition.

    Financial Aid Timeline:

    • August 16, 2025 - Financial Aid Application Deadline
    • August 20, 2025 - Tuition Assistance Notification
      • HYS/PMI will notify you via email of its award on or shortly after this date. You will be invoiced for any additional payments. Everyone will receive an email whether you receive financial aid or not. The email will only go to the PRIMARY contact.
    • September 3, 2025 - Payment of Balance for Tuition Due
      • Your account will need to be paid in full by this date. Payment information will be in the Financial Aid email that you receive.
      • If you are NOT going to accept the financial aid and will be pulling out of the program, you must notify HYS by either emailing us at admin@hiyouthsymphony.org or by calling us at (808) 941-9706 to request a refund of your deposit amount.
    • Private Lesson Aid - You will receive an email regarding private lesson aid sometime in September or October. We need to wait until all of our programs have completed registration before determining the amount of aid families will receive for private lessons.

  • Hawaii Youth Symphony (HYS) Consent & Student Information Forms

    All HYS students must have a completed HYS Consent & Information Forms digitally signed by a parent/legal guardian in order to participate in the program.
  • These forms provide consent for all participants who engage in activities with the Hawaii Youth Symphony (also referred to as "HYS" in these documents), a not-for-profit located in the State of Hawaiʻi.

    All of the forms below must be completed BEFORE students can participate in a Hawaii Youth Symphony program. Refunds (partial or whole) will not be issued if a child cannot participate due to these forms not being filled out prior to the start of the 2025-2026 season.


  • Transportation, General Rules, Regulations & Health Procedures Information & Consent

  • Transportation Consent

    Parents/Guardians are solely responsible for taking their children directly to designated Hawaii Youth Symphony programs, as well as picking their children up directly from Hawaii Youth Symphony programs. Hawaii Youth Symphony shall not be responsible for transportation or daycare of children outside of designated program times.

  • Acceptance of the General Rules, Regulations and Health Procedures - See Appendix A & B Below

    APPENDIX A: GENERAL RULES AND REGULATIONS

    We expect students of Hawaii Youth Symphony (“HYS”) to behave in a manner that respects the rights of others and brings honor to HYS and dignity to themselves. Because we are guests at every rehearsal, concert, or studio facility, please leave any area in a condition cleaner and more orderly than you found it.

    In addition, please adhere to the following rules:

    1. Treat all faculty, staff, volunteers, vendors, and school personnel with respect.
    2. No drinks (except bottled water) or gum are allowed in any of the rehearsal/concert/filming facilities. Food is only permitted if and when provided by HYS.
    3. No usage of tobacco products, or e-cigarettes/vapes, or other intoxicating substances.
    4. No gambling or stealing.
    5. No bullying, harassment, or hazing.
    6. No selling, promotion of, possessing, consuming, or being under the influence of drugs, narcotics, hallucinogens, or alcohol.
    7. No socializing with outside guests during rehearsal or performance hours.
    8. No texting or messaging, phone or video calls, live streaming, or use of any electronic devices during rehearsals, performances, filming session or recording session.
    9. No sharing of food or drink.
    10. Students are not permitted to swim, surf, or participate in aquatic activities.

    In any case of misconduct, Hawaii Youth Symphony will enforce immediate disciplinary action, which may include expulsion from HYS. The Hawaii Youth Symphony will cooperate fully with law enforcement officials for all transgressions and investigations.

    APPENDIX B: HEALTH PROCEDURES

    In efforts to curb the spread of illness, HYS requires all students who participate via in-person programs to agree to the following:

    1. Stay home when you don’t feel well.
    2. Masks are encouraged but optional for all students, faculty, staff, and volunteers.
    3. Immediately report any COVID-19 symptoms or illness to HYS.
    4. Any student who experiences symptoms of COVID-19, or any student who becomes aware that they were exposed to a known COVID-19 patient should report their condition to HYS immediately via email to admin@hiyouthsymphony.org.

    Additional or Revised Procedures:

    HYS reserves the right to revise its protections, as warranted (such as by new information from the CDC or local/governmental agencies), with/without advance notice.

    Information We Collect and How We Use It
    We may collect email and personal information through the use of third-party vendors, such as vendors that utilize and manage student and program management software. We may also collect email and personal information from our donors as part of our donor database This includes identifiable information used to access our website, such as cookies. This also includes information such as, but not limited to, identifiable information such as names, phone numbers, mailing addresses, and email addresses. We protect our personal information database by means of various technical and procedural measures, and we restrict access to information by unauthorized persons. We require that any third-party service providers comply with confidentiality requirements and to not use your personal information for marketing purposes without your consent.


    We reserve the right to use personal information to protect the security or integrity of our Website; to protect against a threat to personal safety or destruction of property; to protect against legal liability; or to cooperate with government officials or parties in litigation, or as otherwise required by law. We may also share personal information with our successors and assigns in accordance with law.


    COPPA (Children Online Privacy Protection Act)
    When it comes to the collection of personal information from children under the age of 13 years old, the Children's Online Privacy Protection Act (COPPA) puts parents in control. The Federal Trade Commission, the United States' consumer protection agency, enforces the COPPA Rule, which details what operators of websites and online services must do to protect children's privacy and safety online.


    We do not specifically market to children under the age of 13 years old.

  • Release of Liability Form

    PLEASE READ THIS DOCUMENT CAREFULLY. BY SIGNING IT, YOU ARE GIVING UP LEGAL RIGHTS.
  • In consideration for becoming involved in Hawaii Youth Symphony’s programs and the related activities (collectively, “activities”) conducted by and at Hawaii Youth Symphony, {nameOf113}, hereby agrees as follows:

    ASSUMPTION OF RISK: I agree that my child/ward is voluntarily participating in the activities offered by Hawaii Youth Symphony including, but not limited to, the use of the equipment, facilities, and the premises. I am assuming on behalf of my child/ward, all risk of personal injury, bodily harm, disease, dismemberment, death, or disability to my child/ward that may result from participation, or any damage, loss or theft of any personal property which my child/ward may incur. I understand that Hawaii Youth Symphony provides private and group music lessons and performances, and may be driving my child/ward around in the community and taking them to and from places including, but not limited to, rehearsals, concerts, housing, and recreational facilities.

    RELEASE OF LIABILITY: I understand that my child/ward will be engaging in group activities, and individual lessons in the community and at facilities and premises provided by Hawaii Youth Symphony. I agree on behalf of myself and my child/ward and our personal representative, successors, heirs, and assigns to hold Hawaii Youth Symphony, and its affiliates, instructors, officers, directors, agents, employees, licensors, designers, and advisors, as well as the property owner and tenants of the property and the owners, manufacturers and installers of the equipment comprising the Hawaii Youth Symphony facility/vehicles (collectively, the “Releasees”) harmless from any and all claims or causes of action arising out of my child’s/ward’s participation in Hawaii Youth Symphony programs.

    I expressly release and forever discharge Releasees from any and all liability, claims, demands or causes of action whatsoever arising out of any damage, loss, personal injury, disease, death to my child/ward, or damage to instruments or equipment, while participating in any of the activities offered by Hawaii Youth Symphony. This includes, without limitation, the transportation, receiving of instruction, and any other possible activities that may be involved in Hawaii Youth Symphony’s programs. This release is valid and effective whether the damage, loss, or death is a result of any act or omission on the part of any Releasees or from any other cause. This Waiver and Release of Liability includes, without limitation, injuries, or accidents, which may occur as a result of the: (a) use or misuse of the facility in any way by anyone, (b) use of any equipment that malfunctions or breaks, (c) improper maintenance of the facility, grounds, or any equipment, (d) instruction or supervision (e) slipping, tripping and/or falling while in the facility or on the surrounding premises (f) transportation of your child/ward by Hawaii Youth Symphony staff or volunteers. This Release of Liability also expressly includes a release for any and all claims arising out of
    or under applicable Hawaii laws.

    I understand that I am voluntarily giving up my and my child’s/ward’s right to bring a lawsuit or claim against the above-mentioned Releasees.

    I give HYS permission to make, adapt, publish, distribute, publicly perform, transmit, display and/or otherwise use audio and video recordings and photographic images of my child for archival, fundraising and publicity purposes, in all media now known or hereafter invented, worldwide, royalty-free, and in all fields of use. I hereby release HYS and its audio and video recorders and photographers from any and all liability for use of my child's publicity rights, including my child's name, voice or likeness, whether alone or in composite form. I understand that the audio and video recorders, photographers and HYS can use any artistic procedure for composition needed to produce a product, publication or display, for promotional (including fundraising) reasons.

    I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF ALL LIABILITY AND A WAIVER OF ANY RIGHT THAT I MAY HAVE ON BEHALF OF MYSELF AND/OR MY CHILD/WARD TO BRING A LEGAL ACTION OR ASSERT A CLAIM FOR INJURY OR LOSS OF ANY KIND AGAINST HAWAII YOUTH SYMPHONY, SHOULD ANY CLAIM BE MADE, I UNDERSTAND AND AGREE THAT I WILL BE RESPONSIBLE FOR ALL ATTORNEY’S FEES AND DEFENSE COSTS INCURRED BY HAWAII YOUTH SYMPHONY, IN CONNECTION WITH OR IN THE DEFENSE OF THAT CLAIM.

    This Release and Waiver will be in effect from the date signed. I have read the above, considered its effects, understand its content and agree, on behalf of myself and my child/ward, to the terms as stated above. This agreement specifically contains an indemnity agreement whereby I agree to reimburse the Releasees against any damages (including attorney’s fees and costs) incurred as a result of any lawsuit, claim, or action brought by myself, my child/ward, or any other party, related in any way to my child’s/ward’s involvement with Hawaii Youth Symphony.

    Please make sure you fill out this section carefully and accurately. We do provide food at some events for students. Allergy/dietary information is very important to include here.
  • If you choose NOT to give us the medical information below, HYS staff and emergency personnel may not be able to adequately help your child in the case of an emergency. HYS will not be liable for information that is not provided on this form.

    We are asking for medical information in case your child has a medical emergency during a rehearsal or event. We also sometimes provide food for students and need to know if your child is allergic to anything or has any dietary restrictions. This information will only be shared with HYS staff.

    I hereby give my consent to any emergency facility and/or physician to administer any treatment that such facility or physician may deem necessary or appropriate to my child. In the event of an emergency during which I cannot be reached, I give consent to transport by ambulance if the situation warrants it.

    I understand that I will be responsible for all expenses associated with above medical treatment. I will defend, indemnify and hold Hawaii Youth Symphony and its officers, directors, agents, volunteers and attorneys harmless from and against any damages, liabilities, claims, costs or expenses arising from any such medical treatment.

    This information is being collected to help us apply for funding and for reporting purposes only. All questions in this section are optional.

  • Volunteers

    We rely on volunteers to help at many of our events. If you are interested in volunteering, please answer the following questions. We will add you to a list to receive information on volunteer opportunities. This section (except the first question) is optional.
  • Types of Volunteers:

    • Ushers: People to help watch the doors, pass out programs, help direct guests to bathrooms. We ensure that parents get to watch their child perform as we usually have different shifts for this. Usually need to meet about 1 hour before the event starts.

    • Merchandise Sales: People to help sell HYS merch. Usually before the event starts, during intermission and after the event ends. Parents will get to see their child perform. These volunteers usually need to meet about 1 hour before the event starts.

    • Parent Chaperones: People to help at concerts or events to watch students backstage or in the audience. Parents will be able to watch their child perform. All chaperones need to complete a background check and volunteer training with HYS.

  • If you applied for TUITION ASSISTANCE FINANCIAL AID:

  • You indicated that you are a NEIGHBOR ISLAND student who will be flying to Oahu for rehearsals. Neighbor Island students get an automatic 50% discount on tuition. Please make sure you:
